Internship Learning Agreement

The Internship Learning Agreement is completed by the student, in collaboration
with the faculty sponsor and site supervisor. The Agreement serves several
purposes:
Job Description (attach additional sheet
as necessary)
What tasks and projects will you be responsible for at this internship? Please
list/describe.
Educational Objectives (attach additional
sheet as necessary)
What do you want to gain from this internship? For example, do you hope to
explore career possibilities, develop new skills, test theories learned in
class, etc.? Please list/describe.
Evaluation Requirements (attach additional
sheet as necessary)
How will you present to your faculty sponsor that you accomplished the goals
outlined in the "educational objectives" section? Examples include a
journal, samples of work completed, a paper, etc.
Please obtain signatures of all learning partners and be sure all parties have
a copy of this Agreement:
